Another reason I opted out is because as a Nurse Practitioner I'm only reimbursed 85% of the physician rate, but I spend far more time with my patients, and have similar if not better outcomes. Now I only do fee for service, and I don't have to spend half my day arguing for payment, and I see fewer patients. I also offer a sliding scale for those that may need it.
